Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our pledge regarding your protected health information (PHI)
Louisiana Medical Tech Group is required by 45 CFR Part 164 (HIPAA Privacy Rule) to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your PHI
- Treatment. We may share your information with other clinicians involved in your care, including referrals to specialists, hospital admissions, and laboratory orders.
- Payment. We may use and disclose PHI to bill your health plan or another responsible party.
- Health care operations. Quality improvement, peer review, accreditation, audit activities, and credentialing.
- Required disclosures. Public health reporting (e.g. communicable diseases to LDH), judicial orders, abuse/neglect reporting.
Your rights under HIPAA
- Right to inspect and copy your medical records (typically within 30 days)
- Right to amend incomplete or inaccurate records
- Right to an accounting of certain disclosures (excluding TPO)
- Right to request restrictions on disclosures
- Right to confidential communications via alternative means or at alternative locations
- Right to a paper copy of this notice at any time
- Right to file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights
Breach notification
If there is a breach of your unsecured PHI, you will be notified in writing within 60 days of discovery, as required by 45 CFR 164.404.
